What Is Form 1040 Schedule 6?

On page one of IRS Form 1040, the home address line, the taxpayer is asked to attach Schedule 6 if they have a foreign address. Form 1040 Schedule 6, Foreign Address and Third Party Designee, was created as part of the Form 1040 redesign implemented for the 2018 tax year. You do not file IRS Schedule 6 with the older 1040 series forms such as Form 1040A or Form 1040EZ.

Form 1040 Schedule 6, Foreign Address and Third Party Designee, asks that you complete the Foreign Address line if your home address is located outside of the United States of America.
